Gathering Information When there are concerns about a child's development, call us at 252-0415 or e-mail us at ecassessment@isd742.org. Describe the concern and give us the child's name, date of birth, parent name(s), address and phone number. Within two weeks we'll conduct a phone interview or home visit to gather additional information about your child's medical history and skills in all areas of development. The Planning Meeting The gathered information is shared with the entire Early Childhood Assessment Team. The team determines if an assessment is necessary and, if so, which assessments should be administered. Assessing Your Child If further assessment is necessary, an assessment coordinator is assigned. That person contacts parent(s) to schedule appointments. Typically, children ages 0-2 are assessed at home. Children ages 3-5 are usually assessed at Roosevelt Early Childhood Center. There is no cost. Download our Assessment Areas Guide for details about the information we gather during the assessment process. Determining Eligibility When the assessment is complete, we meet with you to discuss the results and to determine if your child is eligible for, and in need of, special education services. A Student Services representative attends the meeting. You are welcome to invite others as well. Your Child's Plan If your child qualifies for special education services, we write a plan of action that sets goals and objectives for meeting your child's needs. Typically, an Individualized Family Service Plan (IFSP) is written for children ages 0-2. An Individualized Education Program (IEP) is written for children ages 3-5. Services are provided based on your child's needs.
